Advance Health Care Directive

An Advance Health Care Directive (commonly referred to by the acronym "AHCD") allows another person (your health care agent) to act on your behalf to make medical decisions for you when you are unable to make those decisions yourself.  Although an Advance Health Care Directive does not deal with how your property is distributed following your death, it is perhaps the most personal of the estate planning documents.
